If a paper or otherwise manual verification is required by another agency: Send the required form, request, and $25 money order to the LSBN office via mail at 17373 Perkins Rd, Baton Rouge, LA 70810. The form will be completed after receipt of the above documents and the completed form will be mailed to the agency indicated.
